how to open a home care respiratory care business?

I am a therapist and wants to open a service like other therapists who go to patients who need ventilation to provide care as "home visits" to the nurses. "First, as we expect to be paid? Most expect that Medicare or insurance coverage. You must have a Medicare provider number Medicare Bill will provide hub equipment, etc, which are referrals do not have a plan business? that insurers accept billing? have sufficient cash reserves to last until the payment? insurance companies and Medicare does not pay within 30 days. maintain the equipment?
